The first time I came across an adoption contract was when my husband and I adopted an akita from a breed rescue. The more I thought about the contract, the more I liked it. When I started doing reptile rescue and rehoming, I came across others doing the same thing, and I would run into people who had adopted reptiles from some of these rescues. I was surprised at how ill-prepared these adopters were when they got the animals, not having been screened at all. So, I came up with an adoption application, used to try to get a feel for what the prospective adopter does and doesn't know, and to really get the person to think about things they probably hadn't already thought about in terms of taking on the responsibility of the desired species. I also developed an adoption contract, for use when an adoption takes place. I am no longer doing rescues, so have no herps for adoption, but I have always made my application and contract forms available to anyone who wants to use them. So, feel free to read them, use them as is, or modify them to fit your particular situation. Adoption application: HTML, Text
